WHAT IS CONNECTED CARE?<\/strong><\/p>

Our program is a Medicare service for patients diagnosed with chronic conditions like overactive bladder, urinary incontinence, chronic cystitis, and pelvic organ prolapse.<\/p>

As part of our Connected Care program our dedicated Care Team will check-in with you over calls and texts each month to see how you’re doing and help you with between-visit-needs like medication refills, scheduling, symptom questionnaires and more.<\/p>

WHAT IS THE GOAL OF THE PROGRAM?<\/strong><\/p>

We believe that by proactively engaging patients each month we can help them improve their health and better manage their conditions. This program is not a substitute for regular office visits. Rather, the program is an extension of our Care Team\u2026to help patients be more connected to their providers and in more control of their health.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

Once you provide your consent to be part of our program, your healthcare provider will assign a care plan that is specific to your conditions and health goals. Our Care Team will use that care plan to proactively manage your health each month.<\/p>

\u2022 General health assessments through calls and text messages
\u2022 Education on best practices for managing your specific conditions
\u2022 Weekly health reminders, tips, and assistance with questions
\u2022 Medication Oversight
\u2022 Preventative Care Planning
\u2022 Coordinating and preparing for upcoming visits<\/p>

Our Care Team will take the lead in reaching out to you each month. All you need to do is engage and participate in the program.<\/strong><\/p>

Here’s what we ask of you…<\/strong><\/p>

\u2022 Speak briefly with our Care Team each month to update them on your conditions.<\/strong><\/p>

\u2022 Respond to text messages from our Care Team so they can track the progress of your care plan.<\/strong><\/p>

It’s that simple! By investing just a few minutes of your time each month with our Care Team we are confident we can help you improve your conditions and achieve your healthcare goals.<\/p>

Most importantly, our Connected Care program is designed to prevent you from having problems and potential complications, keeping you out of the hospital and away from preventable medical expenses. Like any other office visit, medication, or procedure, there may be a small co-pay due if you haven’t already met your deductible. If you have supplemental insurance these co-pays are usually fully covered. The money you’ll save with our program will more than cover any small copays that might be due. If at any time you don’t find the program valuable just let us know!
